Surgical Emphysema without Pneumothorax: A Diagnostic Dilemma

Surgical emphysema, also widely known as subcutaneous is defined a clinical state in which air gets trapped subcutaneously. The etiology may be traumatic or atraumatic, and multiple causes each are present.
 Case report. A 63-year-old female patient presented to the emergency room with facial soft tissue injury following road traffic accident developed surgical emphysema later on no evidence of rib fractures pneumothorax. Infectious was ruled out. Due respiratory embarrassment, electively intubated. Conservative management provided. Emphysema resolved within 24 hours after accident.
 Discussion. Various theories have been outlined literature, like Mackler effect, Crampton theory ballvalve mechanism. In our case, it difficult apply any these confirm diagnosis. still used treatment emphysema.
 Conclusion. common encounter trauma practice, its identification essential rule out lead distress.
